Terms and Condition
Effective Date: May 8, 2025

These Terms and Conditions (“Agreement”) govern the provision of services by GeneTech

Diagnostics (“we,” “us,” or “our”) and apply to all clients (“you” or “Client”) who schedule or

utilize our DNA or Drug specimen collection services.

1. Scope of Services

GeneTech Diagnostics provides specimen collection services only for DNA and Drug

testing purposes. We do not conduct laboratory testing or interpret test results. All

specimens are collected following industry protocols and are submitted to certified

laboratories for analysis.
 

2. Payment Policy

• Full payment is required at the time of booking to confirm any appointment.

• Accepted forms of payment include credit/debit cards and Paypal.

• Appointments without prior payment may be cancelled at our discretion.
 

3. Cancellations, Refunds & Rescheduling

• All sales are final. No refunds will be issued under any circumstances.
 

• Appointments may be rescheduled once at no additional cost if notice is provided

at least 24 hours prior to the scheduled appointment.
 

• Late arrivals, missed appointments, or rescheduling requests made less than 24

hours in advance will result in forfeiture of payment and require a new booking and

payment.
 

4. Client Responsibilities

You agree to the following:

• Arrive promptly and provide valid, government-issued identification.• Ensure all individuals being tested are present, cooperative, and properly

documented.
 

• Confirm that all submitted information (e.g., names, dates of birth, case references)

is accurate and complete.
 

5. Privacy & Confidentiality

We value your privacy.

• All personal and case-related data are kept confidential and stored securely.
 

• Test results and case information will only be shared with authorized individuals or

entities as permitted or required by law.
 

6. Limitation of Liability

• GeneTech Diagnostics shall not be held liable for any lab processing errors,

delays, shipping mishaps, or interpretation of results.
 

• Our role is strictly limited to professional specimen collection.
 

• Clients assume all risks related to lab testing, custody matters, or legal disputes

that may arise from test outcomes.
 

7. Compliance with Laws

Our operations adhere to applicable federal, state, and local regulations. This Agreement

shall be governed by the laws of the State of Louisiana, and any disputes shall be resolved

in its courts.
 

8. Modifications

We reserve the right to update or modify these Terms and Conditions at any time.

Continued use of our services after any changes constitutes your acceptance of the

revised Terms.
